We participate with the health insurance plans listed below and we will bill your insurance company for you. If your plan has a co-pay, this will be collected at the time of service. We will also be billing you (the patient) for any part of your charges that are your responsibility such as deductible, co-insurance or services that your insurance does not cover. Most HMO plans will require a referral from your PCP prior to being seen in your office. We recommend that you verify with your plan if this is an initial requirement for specialist treatment.
Please bring your insurance card with you to every visit. We will need to scan your insurance care into our system in order to effectively bill your insurance. Annually we will have you complete a personal information form and an assignment of benefits allowing us to bill your carrier and receive payment for services in our office.
